2/12/2023 came for return visit front desk receptionist refused me an appointment due to doctor not in. I ask to speak to Nurse Practitioner and she clear me to see her for refills meds. After sign in with portal the front desk allow three patients ahead of me after I turn in computer and when I?? Her she said I had only 1 pt ahead of me. She then got loud and unprofessional because she told me another patient would be swnd before me again and said we think we should be treated like VIP but it don’t work like that she was tired of everyone coming in complaining. She spoke with register nurse and found out they were having computer issues with new system. Please speak with Dr Victor to have these issues addressed at Fairfield AL AFC everyone else were nice in the back pt care area. Over charged at visit and never received refund

I was scheduled by an employer to visit this location for a pre-employment hair drug test. Upon arrival to this location, I waited an hour to be seen. I understand there is a nationwide staffing shortage, but waiting an hour for a process that should take five minutes is a bit concerning for this location.
After I was taken into the exam cubical that was rather small, two assistants (Madison Carmichael , who is listed on my paperwork and another who’s name I did not receive) crowded me. They asked me where I wanted hair extracted from. Due to my ethnicity and background, I can only provide a sample from my head. Although I explained to the staff member that I was sure and that I could not provide a sample from anywhere else, she made a remark that was extremely offensive. She proceed to say, “Are you sure that you want it from your head because you hair is already so thin?”. I took great offense to this comment since I am already suffering from hair loss. This treatment was unprofessional and insensitive.
The two staff members in this room with me consistently argued about the plan of action because they were unsure of the procedure. When I asked how many times they had perform this task, one responded with a couple. Then, she approached my head with scissors while the other one stated that she was not cutting close enough. As the patient, I felt uncomfortable with this testing process. The supposed professionals questioned each other too many times about how to perform this task. The first sample came from directly where the part in my hair ended, leaving a bald spot. Afterwards, I was asked if I wanted the next one from the same spot. The staff members had an exchange of how the sample has to be from different places. After the second sample was taken, I felt as if it had been from the same location. The bald spot left was the size of a quarter which I confirmed after the appointment. I inquired about where the second sample specifically came from, but received a false reply. The worker claimed they extracted it from a different, undisclosed spot. The worker started an extremely rude and unprofessional argument with me. She attempted to tell me that both samples were not from the end of my part even though I could feel the bald spot and later saw it in a picture taken by a friend.
My overall experience with this location was less than ideal. The rude employees, cramped testing room, and untrained workers made my visit unpleasant and uncomfortable. My concerns with this experience lie with the unprofessionalism of the employees and their visible lack of preparation going into this procedure.

I was denied copies of the paperwork from MY physical. The woman said I was not entitled to them. How?! When they were touching and checking out MY body!!!! But I’m not allowed to have copies of the paperwork or any type of paperwork from my appointment. BIG violation of HIPAA. Denying a patient a copy of any form of paperwork
