How to File a Complaint Against Florida Blue

Members filing a complaint against Florida Blue should collect their member ID, plan name, claim or authorization number, provider information, dates of service, Explanation of Benefits, previous correspondence, and any case numbers before escalating. Health-plan disputes can involve claims, medical coverage, prior authorization, billing, provider access, customer service, discrimination, Medicare benefits, or prescription-drug coverage, so the correct complaint route depends on the issue.

Step 1: Contact Florida Blue Member Services

Explain the concern clearly, provide the relevant claim or service information, and ask the representative to document the issue. Medicare members should instead use 1-800-926-6565. Keep the representative’s name, call date, and any case or reference number provided.

Step 2: File a Formal Grievance or Appeal
